Preparing for Your Hospital Stay (Booklet)

・Please fill in the “Declaration Form”, “Verification of Hospital Admission History ”, and “Application for Patient Wear Rental and Diaper Service” in advance, and bring it with you at the time of your admission.

・The joint guarantor of the Declaration Form is limited to those who can bear the medical expenses etc. in a responsible manner, and who belongs to a different household from the guarantor of the patient’s identity (relatives, acquaintances, friends, siblings in different households, parents, children, etc.).

Medical Consultation Registration Card (Patient Registration Card)

・Please present this card at the Reception Desk for Inpatient Admission .

Health Insurance Card / Other Medical Certificates (medical care for the elderly, welfare medical care, specific diseases, etc.)

  • If you do not show your health insurance card, you will be asked to pay the entire amount at your own expense. Please show your health insurance card at the Reception Desk for Inpatient Admission  at the time of your admission. Even if you have already shown it at the outpatient clinic for that month, we may ask to confirm once again at the time of admission.
  • If you are hospitalized due to a traffic accident or a work-related injury, different types of insurance (automobile liability / occupational accident insurance) may apply, so please consult the Reception Desk for Inpatient Admission .
  • If you have an Eligibility Certificate for Ceiling-Amount Application, the fees will be reduced at the counter. Please show it together with your insurance card.
  • If you have a Standard Cost Reduction Certificate, the cost will be reduced according to the copayment limit, such as for meals. Please show it together with your insurance card.
  • Please refer to this page for the Eligibility Certificate for Ceiling-Amount Application and the Standard Cost Reduction Certificate.
  • If there is a change in your insurance plan (additions, expiration of validity period, retirement, etc.) during your hospital stay, please contact the Inpatient Admission immediately. After claiming insurance, we may not be able to change anything even if you contact us or present us with the documents.
