How Your First Visit Will Go

1.Arrival / Reception

Patients Who Have a Letter of Referral

Patients who made an appointment for the first visit through a medical institution

Please come to the Reception Desk for Patient(s) with Referral [No. 30] on the 1st floor of the Outpatient Building, 20 minutes before the appointed time.
(Depending on the treatment you are scheduled to receive, we may ask you to come to the hospital one hour before the appointed time.)
Monday to Friday, 8:15 a.m.–5:00 p.m.
Saturdays, 8:15 a.m.–12:30 p.m.

Patients other than the above (those who do not have an appointment for the first visit)

Please come to the Reception Desk for Patient(s) with Referral [No. 30] on the 1st floor of the Outpatient Building, open Monday to Saturday from 8:15 a.m. to 11:30 a.m.

What to Bring

Letter of referral (form providing medical information), health insurance card, various medical certificates, appointment slip (for those who have an appointment)
* If you have any test results, image data (CD-R, DVD), or medicine notebooks, please bring them with you.

Patients Who Do Not Have a Letter of Referral

Please come to the Outpatient Desk [No. 31] on the 1st floor of the Outpatient Building, open Monday to Saturday from 8:15 a.m. to 11:30 a.m.
* From Monday to Saturday, the ophthalmology outpatient clinic accepts only new patients with a letter of referral.
* We are available 24 hours a day, 365 days a year for those patients who are brought in by ambulance and those who are in need of emergency care.

What to Bring

* If you have any test results, image data (CD-R, DVD), or medicine notebooks, please bring them with you.

In order to be seen at our hospital, you must fill in the Patient Registration Form.
In order to shorten the time spent waiting at the Reception Desk for Patient(s) with Referral, please download and fill out the Patient Registration Form in advance, and submit it at the Reception Desk for Patient(s) with Referral [No. 30] or the Outpatient Desk [No. 31].
* The forms are also available at the Reception Desk for Patient(s) with Referral , so you may also fill it out upon arrival.

2.Medical Consultation

You will be examined and undergo tests in the relevant clinical department.

3.Billing

First–Time Patients Guide

Health Insurance Card / Various Types of Medical Certificates

A health insurance card is a certificate that shows that you have health insurance.
  • At our hospital, we will check your health insurance card or medical certificate at the time of your first visit and once a month thereafter.
  • Please present your health insurance card and medical certificate to the Reception Desk for Patient(s) with Referral [No. 30] or the Outpatient Desk [No. 31] before your medical consultation.
  • If we cannot confirm the validity of your health insurance card, you may be required to pay the full amount at your own expense.
  • In case you are being seen because of a traffic accident or a work-related injury, automobile liability insurance or occupational injury insurance may apply. Please consult the Reception Desk for Patient(s) with Referral [No. 30] or the Outpatient Desk [No. 31] before your medical consultation.


At our hospital, the following will be considered “first visits.”
  • Patients who are being seen at our hospital for the first time
  • Patients who have been treated at our hospital before, but have already recovered from the illness
  • Even if the patient is being seen for the same disease and symptoms as previously, it shall be considered a first visit if one month or more has passed from the time the patient chose to discontinue treatment.
  • Patients who have not been seen at our hospital for 3 months or more (except patients who have made appointments in advance)

Letter of Referral (Form Providing Medical Information)

Our hospital is an advanced treatment hospital that provides highly specialized and advanced medical care.
Please bring a letter of referral from your family doctor or local medical institution.

Elective Medical Care Fees

If you do not have a letter of referral, you may be charged 7,700 yen (including tax) as the elective medical care fee.

  • Please note that the order in which patients are called may change due to emergencies, and depending on test results, progress of treatment, and the prioritization of patients in worse condition.
  • If you are currently hospitalized at another medical institution, please make sure to mention the fact at the Reception Desk for Patient(s) with Referral [No. 30] or the Outpatient Desk [No. 31] before your medical consultation.

  • Is it possible to be seen at your hospital without a letter of referral?
    It is possible for you to be seen by a doctor. However, we will give priority to those who have made appointments in advance, and those who require more urgent care.
    In addition, you may be required to pay an elective medical care fee of 7,700 yen (including tax), together with the regular medical fees.

    * From Tuesday to Saturday, outpatient services in ophthalmology are open only for first-time patients with letters of referral.
    * We are available 24 hours a day, 365 days a year for those patients who are brought in by ambulance and those who are in need of urgent care.
